This is the question we are asked most often by parents in the US, so to answer it plainly: the medium of instruction is English. Live classes, notes, tests, doubt sessions and recordings are all in English, and no Hindi is required at any stage. A student who has never studied in India sits in the same batch as everyone else and misses nothing.
NEET itself is offered in 13 languages, and English is one of them — it is the standard choice for NRI candidates and for anyone schooled in an English-medium curriculum, whether that is CBSE abroad, IB, IGCSE, AP or A-Levels. The biology terminology is standard scientific English, the same vocabulary used in IB and AP biology; what changes between those syllabuses and NEET is emphasis and depth, not language.

Most students in the US are not on NCERT. NEET is written entirely against the NCERT Class 11 and 12 syllabus, so the real question is not whether your child is a strong biology student — it is which topics their curriculum never asked them to cover, and how NEET's question pattern differs from the one they are used to.
